Required for IRS Pub 4557 preparer due-diligence. Photo of your government-issued ID + your Social Security card (or a clear list of the SSN).

Tip: place the ID flat on a dark surface and photograph from directly above. Don't hold the ID in your hand — IRS-acceptable images require all corners and the magnetic stripe (if any) to be clearly visible.

7a. Healthcare coverage
Did you (or anyone on your return) purchase health insurance through the Marketplace OR receive Advance Premium Tax Credits during the year?

Required for premium tax credit reconciliation. Without the 1095-A, the IRS may delay your return.

7b. Due-diligence questions

Required if claiming dependents, the Earned Income Credit (EIC), Child Tax Credit / Additional CTC, Credit for Other Dependents, American Opportunity Credit (AOTC), or filing Head of Household. Under IRC §6695(g), we are required to ask + document these questions. Answer to the best of your knowledge.

Did the qualifying child(ren) live with you in the United States for more than half of the tax year?
Can anyone else claim the child(ren) as a qualifying child this year?
Do you have records to substantiate residency and relationship for the qualifying child(ren) (school records, medical records, lease, custody documents, etc.)?
If filing Head of Household: did you pay more than half the cost of keeping up the home for the year?
7c. Treasury Offset acknowledgment

The Treasury Offset Program (TOP) may apply your refund against past-due federal/state debts.

I understand that any refund may be applied (offset) against past-due federal/state debts including defaulted student loans, child support obligations, federal/state tax debts, and other amounts owed to government agencies. The IRS Offset Hotline is 1-800-304-3107 for any inquiry about specific debts that may offset my refund.

    Payment policy. I understand that preparation fees are due before my return is completed or e-filed. I will receive an invoice with my flat fee. Fees paid are non-refundable once preparation has commenced.
    Engagement scope. I understand that this engagement includes preparing the current-year return identified above. Services NOT included unless separately engaged: prior-year returns, amended returns from other preparers, IRS examination / appeals / Tax Court representation beyond AFSP scope, ongoing tax planning between engagements, and bookkeeping cleanup.
    Electronic signature consent (E-SIGN Act). I consent to use of electronic signatures throughout this engagement, including for engagement letters, organizers, Form 8879 e-file authorizations, and any other documents requiring my signature. I confirm I can access and retain electronic documents in PDF format.
    Form 8879 acknowledgment. I understand that this organizer submission is NOT my authorization to e-file my return. After my return is prepared, I must review the completed return and sign IRS Form 8879 (and any state e-file equivalents) BEFORE my return can be e-filed.
    9c. §7216 Consent to USE of tax return information

    Federal law requires this consent form be provided to you. Unless authorized by law, we cannot use, without your consent, your tax return information for purposes other than the preparation and filing of your tax return.

    You are not required to complete this form. If we obtain your signature on this form by conditioning our services on your consent, your consent will not be valid. If you agree to the use of your tax return information, your consent is valid for the amount of time you specify. If you do not specify the duration of your consent, your consent is valid for one year from the date of signature.

    I, the taxpayer named on this organizer, authorize CKT Ventures, LLC (doing business as TaxOwl Advisors) to use my tax return information for the following purposes:

    • To identify and offer me additional services TaxOwl Advisors provides, including tax planning sessions, bookkeeping, payroll, sales-tax compliance, and amended returns;
    • To send me seasonal communications about year-end planning opportunities, withholding reviews, and quarterly estimated-tax updates relevant to my specific tax situation;
    • To recommend specific tax-saving strategies based on the data in my prepared return.

    Duration: This consent is valid until the earlier of (a) the date I sign the engagement letter for my next tax-return preparation, (b) the date I revoke this consent in writing (sent to hello@taxowl.co), or (c) one year from the date I sign this consent.

    9d. §7216 Consent to DISCLOSURE of tax return information

    Federal law requires this consent form be provided to you. Unless authorized by law, we cannot disclose, without your consent, your tax return information to third parties for purposes other than the preparation and filing of your tax return. If you consent to the disclosure of your tax return information, Federal law may not protect your tax return information from further use or distribution.

    You are not required to complete this form. If we obtain your signature on this form by conditioning our services on your consent, your consent will not be valid. If you agree to the disclosure of your tax return information, your consent is valid for the amount of time you specify. If you do not specify the duration of your consent, your consent is valid for one year from the date of signature.

    I, the taxpayer named on this organizer, authorize CKT Ventures, LLC (doing business as TaxOwl Advisors) to disclose my tax return information to the following third parties for the following purposes:

    • IRS-authorized e-file transmitters and tax-preparation software vendors as required to e-file my federal and state returns;
    • State revenue authorities for state return processing;
    • Banking institutions for direct-deposit processing and refund-transfer products (if I elect direct deposit or a refund transfer);
    • Other professional service providers I specifically authorize in writing in the future (such as my financial advisor, attorney, or a lender requiring tax transcripts).

    Duration: This consent is valid until the earlier of (a) the date I sign the engagement letter for my next tax-return preparation, (b) the date I revoke this consent in writing (sent to hello@taxowl.co), or (c) one year from the date I sign this consent.
